Before concluding that an organisation possesses an effective early-warning system, it should ask:

4. Are weak signals preserved?

5. Are near misses systematically captured?

6. Are potential consequences assessed?

7. Is the organisation dependent upon luck or informal intervention?

8. Are control-stress indicators monitored?

9. Are reporting-confidence signals considered?

10. Are safeguarding warnings capable of overriding ordinary thresholds?

11. Are vulnerabilities assessed cumulatively?

12. Can related signals be connected across systems?

13. Is deterioration velocity measured?

14. Are rate-of-change triggers established?

15. Are warning thresholds genuinely preventative?

Before people stop reporting, there may have been delays, inaccessible processes and declining confidence.

And before institutional failure becomes visible, there may have been many people who already knew that something was beginning to go wrong.
