PART 1 — THE REBUILD SELF-ASSESSMENT

This is not therapy.

It is orientation.

It helps a person understand:

Where am I?
What stage am I in?
What do I need right now?

SECTION A — Daily Stability Check

Rate each from 1–5
(1 = Not Stable | 5 = Fully Stable)

□ I am sleeping at least 6 hours consistently
□ I am eating at least 2–3 meals per day
□ I feel physically safe where I am staying
□ I can manage basic daily tasks (washing, cooking, cleaning)
□ I am able to care for my children consistently
□ I can concentrate for 30+ minutes at a time
□ I feel emotionally regulated most days

Score Range:
0–14 = Stabilisation Required
15–25 = Functional but Fragile
26–35 = Stable Foundation

If Stabilisation Required → focus on safety & support first.
If Functional → begin documentation and advocacy preparation.
If Stable → advance to structured action planning.

SECTION B — Legal & Administrative Position

Tick what applies:

□ I have obtained medical documentation
□ I have copies of key documents (ID, tenancy, financials)
□ I have logged key incidents chronologically
□ I understand my immediate legal options
□ I have sought initial legal advice

PART 2 — THE REBUILD CHECKLIST (Structured Pathways)

SAFECHAIN™ divides rebuilding into three phases.

PHASE 1 — Stabilise

Focus:

□ File formal complaint (if required)
□ Seek legal representation
□ Apply for relevant protections
□ Prepare structured witness summary
□ Explore therapy / support services
□ Begin financial rebuilding plan

PART 3 — TRAUMA-INFORMED OPERATING RULES
